New Patient Registration
Begin the registration process by filling out the new patient, medical history form. This minimizes your time at our office and allows you to fill out forms at your convenience.

Children’s Care
Start your child’s dental care at the age of one and continue with checkups every six months. To give your child’s teeth maximum protection, our staff applies fluoride varnish when indicated.
Adult Care
Receive detailed screening exams for gum disease, infection, cavities, and traumatized teeth. Options are always given for any recommended treatment. We also answer any questions you have about the pros and cons of any dental treatment. Learn more about our low-radiation X-ray technique.
Insurance
We accept most insurance plans, contact our office to verify acceptance of your plan. We will be happy to file your insurance claims for you; however, your co-payment is due at the time of service. For those services not covered by insurance, we allow a 5% discount for payment-in-full at the time of service with check or cash. We offer a 2% discount with payment-in-full with a credit card.

Billing
As a no-surprise practice, our office always provides you with a printed estimate of the cost of your care before you leave. Cash, credit cards, and checks are accepted, as well as a flexible payment plan. Please contact our financial coordinator for details or see below.
